Idlewild Kennels Health Guarantee and Contract
With this purchase agreement it the understanding that the puppy:

We are purchasing from Idlewild Kennels, Pam Foster, is AKC Limited Registration with restrictions on breeding and show. I understand that I am required to have the puppy spay/neutered by the age of 8 months (spay/neuter contract attached) (Full Registration no spay/neuter contract required)
I have agreed to purchase a puppy from Idlewild Kennels with the full knowledge and understanding of the health guarantee as follows:
the puppy we purchase has received at least three de-worming and has been vaccinated against Bordetella, canine distemper virus, canine adeno-virus type 2(hepititas), canine parvo-virus, and canine para-influenza virus.
The puppy must be seen by a licensed veterinarian within five days of purchase for a physical and fecal examination.
Upon examination, any serious health problems must be reported to the breeder within 24 hours of examination. A signed statement from the veterinarian explaining the details of the problem must be provided to the breeder upon request.
If a puppy is returned after the initial examination (within 5 days) with a serious health problem, Idlewild Kennels will replace or allow full credit toward another puppy from Idlewild Kennels of equal or greater value, as soon as a puppy becomes available. Replacement when available or credit will be given provided the terms and conditions of this health guarantee were followed and a signed certificate from the veterinarian be presented showing regular treatments, proper vaccination given and an explanation of cause of death.
FEEDING INSTRUCTIONS
During the first year, a puppy must be fed quality pet food three times a day. This puppy has been eating _______________________________. It is the responsibility of the buyer to visit the vet regularly and continue with all recommended Vaccination.
I further understand that with the nature of parasites, Idlewild Kennels does not guarantee the puppy will be free of worms or other parasites, including coccidia, or giardia, but that every measure possible has been taken to assure that the puppies will be parasite free.
The seller is not liable for any veterinary costs incurred
The seller sell a puppy as a pet of show/breeding quality prospects, although buyer understands that NO guarantee can be made in this regard.
If the terms and conditions are not fulfilled, this contract is VOID. Buyer agrees that he/she understands this contract fully, and that this contract is under the jurisdiction of the state of Iowa. It is agreed that the place of venue shall be Johnson County Iowa. This document constitutes page 2 of USDA form #7006 with breeder's USDA license number 42A0973.
IDLEWILD KENNELS SPAY/NEUTER CONTRACT

We agree that the dog purchased from seller, Idlewild Kennels will be spay or neutered by the age of 8 months. Origninal AKC Registration application will be mailed at the time proof of spay/neuter has been provided to Idlewild Kennels by a licensed veterinarian. If I fail to do this, or if the dog is bred prior to spaying, I agree to pay Pam Foster, dba Idlewild Kennels, the sum of $1000.00
At no time during the terms of this contract, will the buyer transfer any ownership right to above dog to any other party, unless notified seller ahead of time in writing. If notice is approved by seller, the buyer may proceed with ownership transfer. However, the original contract shall follow the dog and all subsequent owner transfers are subject to and bound by original contract. If the buyer decides to sell said puppy seller shall be given the first right of refusal at the original purchase price.
I believe this to be a legally binding contract under the jurisdiction of the State of Iowa and agree to all terms. It is agreed that the place of venue shall be Johnson County Iowa. This document is page three of USDA Document # 7006 with breeders USDA license number 42A0973
